Duplicate account merge/delete problem solution ?

I have a friend who had a duplicate account 3 years ago,
he got banned for it for 1 year, now he got banned again ( he didn't login to that account, he doesnt' even remember the old account id's
the problem is that he can neither delete old account, nor merge old account or create new account.

its a sticky situation, so is there a solution to it?
and how does one contact mods, if someone gets banned?
so do you think there should be some sort of a system where users can fix the dup account…
