WordPress: Minor Technical issues

Hi there,
Though the icon for the "insert as plain text" is on the new post tool bar, the popup after the clicked turned out all blank and the 'Insert" button on the bottom right hand corner is also missing.
Please find attached the screenshot.
Please assist. Many thanks in advance.
