Need to know what Affiliate Company!?!

Hello, I was just curious about one thing. Awhile back I was on a website where the site had a lock on it and you could only continue by filling out one of the surveys. It was powered by some affiliate program but I don’t recall the name or website of that site. The only name I have in my head is like CPA Live. All help is greatly appreciated! :p
