Want to hire PPC campaign manager

I am interested in hiring ppc campaign manager,but have couple questions.
How ppc manager usually get paid ? I mean is it flat rate, % of the budget or something else
What are average prices for this service and on what it depends ( budget, ammount of keywords, etc? )
Would be thankful for any insight.
