How to sign up Megaupload Rewards Program ?

Dear all,
I em new premium member of Megaupload, when I click on "Rewards", it shows that I have to sign up for Rewards Program. I sign up, and cannot complete the process because one field required is "initials", I don’t understand what to fill in. I sent 3 mails to ask Megaupload but no response. Please advise me !
