Start small grow big, or Start big and grow small ??

What is the best way to work PPC.
A. Start small with 1 adgroup 2 keywords, relevent ad text. Then add more adgroups and keywords ase you grow.
or
B. Start with 10,000 keywords. Group the keywords into smaller adgoups. Let it run for a week or so and then start pausing adgroups and keywords that do not perform.
