How to get mailing list working?

My mailing list is sending people to a 404 error, or so I am told, when they sign up for it.
The sign up sheet I am using is notifylist.
(See blogmethods.com) This is my experiment page, to figure out what is hurting the coding. DO NOT SIGN UP for this email list, nothing will happen. It is a demo to show you what the sign up looks like on my actual website that I put quality time into.
Does anyone else know of a better service that provides mailing lists besides notifylist?
I am getting tired of the negative results in general.
